Apply Imeediately On-LIne by visiting the NCHM website
i. Candidate should have passed 10+2 or equivalent examination from recognized Board with English as one of the compulsory subject.
iii. In case candidate is unable to produce the passing certificate of 10+2 or equivalent examination due to one or other reasons, such candidates may also be allowed to take admission on provisional basis after submission of an undertaking in a prescribed format subject to condition that Passing certificate shall be produced upto 30.09.2019..
iv. Candidate should submit age proof certificate which is given in the secondary passed certificate. The age should not be more than 22 Years as on 1.7.2019 for General /OBC Category and 25 Years as on 1.7.2019 for SC/ST category.
v. In case of filling up of reserved seat of OBC/SC/ST category, category proof certificate from the Competent Authority as per details given in the JEE Information Brochure.
vi. Candidate should submit Medical Certificate from Registered Medical Practitioner in a prescribed format (As in the JEE Information Brochure).
